 Blackthorn Medical Centre

You may have recently received the leaflet ""Better Information Means Better Care" about data extraction from the medical records that we hold about you on our clinical computer system.


Under the Health and Social Care Act 2012 NHS England has the power to direct the Health & Social Care Information Centre (HSCIC) to collect information for all providers of NHS care including GP Practices.  Attached is a patient information leaflet which details what non-patient identifiable information may be extracted from our clinical system. 


You have the right to prevent confidential information about you from being shared or used for any purpose other than providing your care, except in special circumstances.  This will be marked on your medical record.  This will prevent your confidential informaion being used other than when necessary by law (for example if there is a public health emergency).


Your choice will not affect the care you receive. However, you have to actively OPT OUT from the scheme if you do not wish to consent to the data extraction.  If you wish to opt out please contact the practice and ask that your medical records are amended to reflect this and a special code will be added to your medical records that prevents data being extracted.  Please download and completed the Opt Out Form below and return this to the surgery.


This information has also been advertised in the Practice with posters and patient information leaflets.

If you wish to withdraw your consent for data sharing in this instance please download the form below, complete and return to the surgery and your records will be amended.  Please note that each adult will need to withdraw consent for him/herself. 


The data extraction will also apply to children. Therefore parents can sign the form below and withdraw consent on their children's behalf.  Anyone over 16 will need to need to withdraw consent for him/herself.
